Dienogest is a synthetic progestin, which is a type of medication that mimics the effects of the natural female hormone progesterone in the body. It has a high affinity for progesterone receptors and also exhibits antiandrogenic properties.

Indication

Dienogest is primarily indicated for the following uses:

• Treatment of endometriosis: It is used to manage pelvic pain associated with endometriosis.
• Contraception: It is used in combination with ethinylestradiol as a component of birth control pills.
• Heavy menstrual bleeding: When combined with estradiol valerate, it is used to treat heavy menstrual bleeding (menorrhagia) in women who also choose oral contraception.
• Menopausal symptoms: In some countries, it is used in combination with estradiol valerate for menopausal hormone therapy.
• Acne: When combined with ethinylestradiol, it can be used to treat moderate acne in women who also choose oral contraception.

Mechanism of Action

The mechanism of action of Dienogest is multifaceted:

• Progestogenic effect: Dienogest binds to and activates progesterone receptors, leading to changes in the uterine lining (endometrium). In endometriosis, this causes the endometrial tissue to atrophy (shrink), which helps to alleviate pain and reduce lesions.
• Suppression of hormones: Dienogest moderately suppresses the production of estradiol (a type of estrogen), which reduces the trophic (growth-promoting) effects of estrogen on both normal and ectopic (out-of-place) endometrial tissue.
• Antiandrogenic effect: Dienogest has antiandrogenic properties, meaning it can block the effects of androgens (male hormones). This is beneficial in treating androgen-related conditions like acne.
• Ovulation suppression: When used for contraception, it helps prevent ovulation, makes the cervical mucus thicker to block sperm, and alters the uterine lining to prevent a fertilized egg from implanting.
