Description
Tizanidine hydrochloride is a centrally acting skeletal muscle relaxant.
Indication
Tizanidine is indicated for the treatment of spasticity.
- Multiple Sclerosis (MS): It is used to manage the spasticity that is a common symptom of MS.
- Spinal Cord Injury: It is effective in reducing the muscle spasms and spasticity that result from spinal cord damage.
- Cerebral Palsy: It may be used to reduce spasticity in patients with cerebral palsy.
- Stroke: It can be used to treat spasticity that develops after a stroke.
Tizanidine is intended for the short-term relief of spasticity, as its benefits in reducing muscle tone generally outweigh its side effects.
Mechanism of Action
Tizanidine’s mechanism of action is based on its agonistic effect on alpha-2 adrenergic receptors, which are located primarily in the central nervous system.
- Alpha-2 Adrenergic Agonism: Tizanidine acts as a potent agonist at presynaptic alpha-2 adrenergic receptors on spinal cord interneurons.
- Inhibition of Excitatory Neurotransmitters: When Tizanidine binds to these receptors, it inhibits the release of excitatory amino acids (such as glutamate and aspartate) from these interneurons.
Excitatory amino acids are responsible for stimulating the motor neurons that cause muscle contractions. - Reduced Muscle Spasticity: By reducing the release of these excitatory neurotransmitters, Tizanidine effectively reduces the transmission of nerve signals that lead to muscle contraction.
This results in a reduction of muscle tone, spasms, and clonus. - Central vs. Peripheral Action: The central action of Tizanidine is a key differentiator from muscle relaxants that work peripherally. Tizanidine acts on the spinal cord to reduce the fundamental reflex arc that causes spasticity, providing a more targeted effect.
Its ability to reduce muscle tone without significantly impairing muscle strength makes it a valuable tool in the management of spasticity.
