This formulation is designed to provide dual-action relief by targeting both the vascular and inflammatory components of a migraine attack.

Indications

This combination drug is specifically indicated for the acute treatment of migraine attacks, with or without aura, in adults. It is important to note that it is not intended for the prophylactic (preventive) treatment of migraines or for the management of other types of headaches. It should be taken at the first sign of a migraine attack.

Mechanism of Action

The therapeutic efficacy of this combination is based on the distinct yet complementary mechanisms of its two components, working together to provide a more comprehensive and potent effect on migraine symptoms than either drug alone.

1. Sumatriptan Succinate (Triptan Component)

  • Mechanism: Sumatriptan acts as a selective agonist of serotonin receptors, specifically the 5-HT1B and 5-HT1D receptor subtypes. These receptors are found on cranial blood vessels and on nerve endings in the central nervous system.
  • Role in Combination: When a migraine attack occurs, there is a vasodilation (widening) of the cranial blood vessels and a release of pro-inflammatory neuropeptides. Sumatriptan’s action directly counters these effects:
    • Vasoconstriction: By activating the 5-HT1B​ receptors on the intracranial blood vessels, sumatriptan causes them to constrict, which helps to normalize blood flow and relieve the throbbing pain of a migraine.
    • Inhibition of Neuropeptide Release: By activating the 5-HT1D​ receptors on nerve endings, sumatriptan inhibits the release of pro-inflammatory neuropeptides like Substance P and calcitonin gene-related peptide (CGRP), thereby reducing the neurogenic inflammation that contributes to the migraine pain.

2. Naproxen Sodium (NSAID Component)

  • Mechanism: Naproxen Sodium is a non-selective NSAID that works by inhibiting the cyclooxygenase (COX) enzymes, specifically COX-1 and COX-2.
  • Role in Combination: In the context of a migraine, the inflammatory cascade is a key driver of pain and other symptoms. By inhibiting COX enzymes, naproxen sodium prevents the synthesis of prostaglandins, which are potent inflammatory mediators. This action helps to reduce the underlying inflammation and associated pain.

Synergistic Effect

The combination of Sumatriptan and Naproxen Sodium provides a synergistic effect by targeting different aspects of the migraine pathology. Sumatriptan addresses the vascular and neuronal components, while Naproxen Sodium addresses the inflammatory component.
