Description
Rastomine
Description
Rivastigmine, typically formulated as Rivastigmine Hydrogen Tartrate, is a medication in the cholinesterase inhibitor class.
Indications
Rivastigmine is indicated for the symptomatic treatment of cognitive and memory deficits in specific types of dementia.
- Mild to moderate dementia of the Alzheimer’s type.
- Mild to moderate dementia associated with Parkinson’s disease.
Mechanism of Action
Rivastigmine’s therapeutic effect is based on its ability to enhance cholinergic function in the brain by inhibiting the enzymes that break down acetylcholine, a key neurotransmitter.
- The Cholinergic System: Acetylcholine is a neurotransmitter that plays a vital role in cognitive processes such as memory, learning, and attention.
In patients with Alzheimer’s disease and Parkinson’s disease-related dementia, there is a significant deficiency in acetylcholine levels due to the degeneration of cholinergic neurons in the brain.
- Enzyme Inhibition: Rivastigmine works by inhibiting two key enzymes responsible for the breakdown of acetylcholine:
-
- Acetylcholinesterase (AChE): This is the primary enzyme that hydrolyzes (breaks down) acetylcholine in the synaptic cleft.
- Acetylcholinesterase (AChE): This is the primary enzyme that hydrolyzes (breaks down) acetylcholine in the synaptic cleft.
-
- Butyrylcholinesterase (BuChE): This enzyme also breaks down acetylcholine and other choline esters.
It is found both in the brain and in the periphery.
- Butyrylcholinesterase (BuChE): This enzyme also breaks down acetylcholine and other choline esters.
- Pseudo-Irreversible Inhibition: Rivastigmine binds to and inhibits both AChE and BuChE.
The binding is called “pseudo-irreversible” because it forms a carbamate complex with the enzymes that is broken down much more slowly than the binding of a reversible inhibitor. This extended inhibition leads to a sustained increase in the concentration of acetylcholine in the synaptic clefts, which enhances cholinergic neurotransmission.
- Resulting Effect: By increasing the concentration and duration of action of acetylcholine, rivastigmine helps to compensate for the cholinergic deficit, which can lead to a modest improvement in cognitive functions like memory, attention, and learning.
