Indication

Mefenamic Acid is primarily indicated for the short-term treatment of mild to moderate pain. Its key indications include:

  • Primary Dysmenorrhea: It’s one of the most common and effective treatments for menstrual cramps and pain.
  • Musculoskeletal Pain: For the relief of pain associated with conditions like sprains, strains, and other injuries.
  • Other Painful Conditions: It can also be used for the short-term management of mild to moderate pain from various causes.

Mechanism of Action

Mefenamic Acid’s mechanism of action is based on its ability to inhibit the synthesis of prostaglandins, which are fatty acid compounds that act as mediators of pain, inflammation, and fever.

  1. Inhibition of Cyclooxygenase (COX) Enzymes: Mefenamic Acid works by inhibiting the activity of the cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) enzymes. These enzymes are responsible for converting arachidonic acid into prostaglandins.
  2. Blocking Prostaglandin Synthesis: By blocking these enzymes, mefenamic acid reduces the production of prostaglandins throughout the body. The inhibition of COX-2 is primarily responsible for its therapeutic effects, such as reducing inflammation and pain. The inhibition of COX-1 can lead to some of the common side effects of NSAIDs, like gastrointestinal upset.
  3. Unique Antagonism: In addition to its COX inhibition, mefenamic acid is unique because it may also have an antagonistic effect on prostaglandin receptors. This means it not only prevents the production of prostaglandins but also blocks their ability to cause pain and muscle contractions, which is particularly relevant for its efficacy in treating menstrual cramps.
