Description
Levofloxacin is a broad-spectrum antibiotic belonging to the fluoroquinolone class.
Indications
Levofloxacin is used to treat a variety offections, particularly those that are serious, complicated, or caused by organisms resistant to other antibiotics.
- Lower Respiratory Tract Infections: Such as community-acquired pneumonia, nosocomial pneumonia, and acute exacerbations of chronic bronchitis.
- Urinary Tract Infections (UTIs): Including complicated UTIs, acute uncomplicated pyelonephritis (kidney infection), and chronic bacterial prostatitis.
- Skin and Soft Tissue Infections: Such as cellulitis, abscesses, and other uncomplicated skin infections.
- Sinusitis: Acute bacterial sinusitis.
- Anthrax: It is approved for the treatment of inhalational anthrax and post-exposure prophylaxis.
- Plague: It is used for the treatment of plague.
- Bone and Joint Infections: Such as osteomyelitis.
Mechanism of Action
Levofloxacin works by inhibiting two key bacterial enzymes, DNA gyrase and topoisomerase IV, which are essential for DNA replication, transcription, and repair. This is a unique mechanism of action that differentiates it from other antibiotic classes like beta-lactams. The process involves:
- Inhibition of DNA Gyrase: Levofloxacin binds to and inhibits DNA gyrase, an enzyme responsible for unwinding and introducing negative supercoils into bacterial DNA.
This process is necessary to relieve the torsional stress on the DNA helix during replication. By inhibiting DNA gyrase, levofloxacin causes DNA breaks and prevents proper DNA replication.
- Inhibition of Topoisomerase IV: Levofloxacin also binds to and inhibits topoisomerase IV.
This enzyme is crucial for separating the intertwined daughter DNA strands after the DNA has been replicated. Inhibition of topoisomerase IV prevents the final division of the bacterial cell.
- Bactericidal Effect: By disrupting both DNA replication and the separation of new DNA strands, levofloxacin leads to widespread DNA damage and fragmentation, which rapidly kills the bacterial cells.
This makes it a highly potent bactericidal agent.
