Description
The combination product of Ethinyl Estradiol and Cyproterone Acetate is a hormonal medication used for its strong anti-androgenic effects.
Indication
This combination is indicated for the treatment of androgen-dependent conditions in women.
- Severe Acne:
For the treatment of severe acne that has not responded to other treatments, such as systemic antibiotics and topical therapies.
- Hirsutism: For the treatment of moderate to severe hirsutism (excessive body hair growth) in women.
- Androgen-related Alopecia: In some cases, it is used to manage androgen-dependent hair loss in women.
The use of this combination is generally reserved for women who also require oral contraception, as it provides both therapeutic and contraceptive benefits.
Mechanism of Action
The therapeutic effect of this combination is a result of the synergistic and complementary actions of its two components:
1. Ethinyl Estradiol
- Contraceptive Action: As a potent synthetic estrogen, ethinyl estradiol works primarily by suppressing the release of follicle-stimulating hormone (FSH) from the pituitary gland. This prevents the development of ovarian follicles and inhibits ovulation.
- Increase in SHBG: Ethinyl estradiol significantly increases the production of sex hormone-binding globulin (SHBG) by the liver. SHBG is a protein that binds to androgens (male hormones like testosterone) in the bloodstream.
By increasing SHBG, ethinyl estradiol reduces the amount of free, biologically active androgen in circulation.
2. Cyproterone Acetate
- Progestin Action: As a progestin, cyproterone acetate contributes to the contraceptive effect by suppressing the luteinizing hormone (LH) surge, which is essential for ovulation. It also thickens cervical mucus, making it hostile to sperm.
- Anti-Androgenic Action: This is the most critical component for the treatment of androgen-related conditions. Cyproterone acetate is a potent anti-androgen with a dual mechanism:
