Description

Erdosteine is a synthetic oral medication that acts as a mucolytic agent and is a derivative of the thiol-containing compound, acetylcysteine. Unlike some other mucolytics, erdosteine is a prodrug, which means it is pharmacologically inactive in its original form. After administration, it is metabolized in the body into active metabolites that are responsible for its therapeutic effects. It is primarily used for its mucolytic, antioxidant, and anti-inflammatory properties.

Indication

Erdosteine is indicated for the treatment of various acute and chronic respiratory diseases where thick, viscous mucus is a clinical problem. Its main indications include:

  • Chronic Obstructive Pulmonary Disease (COPD): It is used as an adjunctive treatment to reduce the frequency and severity of exacerbations.
  • Chronic Bronchitis: It helps to thin and loosen bronchial secretions, making them easier to clear.
  • Bronchiectasis: It is used to manage the buildup of mucus in the airways.
  • Acute Respiratory Infections: It can be used as a mucolytic to help clear mucus in acute infections.

Mechanism of Action

Erdosteine’s mechanism of action is multifaceted and relies on the actions of its active metabolites. The process is as follows:

  1. Prodrug Activation: After oral administration, erdosteine undergoes rapid first-pass metabolism in the liver. This metabolic process cleaves the thiolactone ring of the parent drug, converting it into at least two active metabolites containing free sulfhydryl (-SH) groups.
  2. Mucolytic Action: The free sulfhydryl groups of the active metabolites are the key to its mucolytic effect. Similar to acetylcysteine, these sulfhydryl groups are able to attack and break the disulfide bonds that hold together the glycoproteins (mucin) in mucus. By breaking these bonds, erdosteine’s metabolites reduce the viscosity and elasticity of the mucus, making it thinner and easier to cough up and clear from the airways.
  1. Antioxidant Effects: The sulfhydryl groups also provide antioxidant protection. They can scavenge reactive oxygen species (ROS) and free radicals, which are generated in the airways during inflammation and contribute to tissue damage. By neutralizing these harmful molecules, erdosteine helps to protect the respiratory tract from oxidative stress.
  1. Anti-inflammatory Properties: Erdosteine has been shown to have anti-inflammatory effects. It can reduce the adhesion of neutrophils to the bronchial epithelium and inhibit the release of pro-inflammatory cytokines, which are key players in the inflammatory response of chronic respiratory diseases like COPD.
